Key Responsibilities
- Perform initial and ongoing assessments of patients and families to determine home care needs
- Develop, implement, and update individualized care plans in collaboration with patients and families
- Administer prescribed medications and treatments, providing direct patient care in accordance with state regulations
- Educate patients and families on health care needs, safety measures, and discharge planning
- Act as Case Manager for assigned caseload, coordinating all aspects of patient care
- Document clinical notes, communicate with physicians regarding patient status, and follow up on physician orders
- Participate in on-call and holiday duties, ensuring continuity of care and proper resource availability
- Supervise and evaluate home health aide services, ensuring compliance with regulations
- Arrange for necessary equipment and services to support patient care in the home setting
Required Qualifications
- Graduate of an accredited school of nursing
- Current licensure in the state of Vermont
- CPR certification
- 1-2 years of experience in acute care or related settings preferred
- Valid driver’s license with reliable, insured vehicle
- Ability to perform patient lifts, transfers, and other physical requirements
- Excellent communication, problem-solving, and observation skills
- Knowledge of infection control and exposure precautions
Preferred Experience / Skills
- Community health or hospice nursing experience
- Experience working independently in home health settings
- Strong organizational skills and capacity to manage multiple patients
