Key Responsibilities
• Provide patient assessments and develop individualized treatment plans based on medical diagnoses.
• Implement therapeutic interventions to improve mobility, strength, and function.
• Monitor and document patient progress, adjusting treatment plans as necessary.
• Collaborate with healthcare team to coordinate patient care.
• Educate patients and families on exercises and health management strategies.
• Ensure compliance with all regulatory standards and facility policies.

Required Qualifications
• Valid state licensure or license eligibility as a Physical Therapist.
• Current CPR/BLS certification.
• Strong knowledge of physical therapy practices and procedures.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Preferred Experience / Skills
• Prior experience in outpatient or rehabilitative settings.
• Familiarity with electronic health records (EHR) systems.
• Bilingual abilities are a plus.
