Job Summary:
We are seeking a compassionate and skilled Licensed Vocational Nurse/Licensed Practical Nurse to join our team in Long Term Care/Senior Living Facilities. As an LVN/LPN, you will provide high-quality patient care, support, and guidance to our residents, while working collaboratively with our interdisciplinary team to ensure optimal outcomes.
Key Responsibilities:
• Conduct thorough patient assessments, develop and implement individualized care plans, and monitor progress • Administer medications, treatments, and therapies as ordered by physicians • Provide wound care, dressing changes, and other specialized care as needed • Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to coordinate patient care • Educate patients and families on health promotion, disease prevention, and management of chronic conditions • Maintain accurate and up-to-date patient records, including documentation of treatments, medications, and progress • Participate in quality improvement initiatives and contribute to the development of policies and procedures • Demonstrate empathy, respect, and compassion in interactions with patients, families, and colleagues
Requirements:
• Current licensure as an LVN/LPN in the state of [State] • Minimum 1-2 years of experience in long term care/senior living facilities • Strong communication and interpersonal skills • Ability to work collaboratively as part of an interdisciplinary team • Basic life support certification (BLS) and ability to obtain advanced life support certification (ACLS) within 6 months of hire • Ability to lift, transfer, and position patients as needed • Familiarity with state and federal regulations related to long term care
