Key Responsibilities
• Provide physical therapy assessments and develop treatment plans for patients in skilled nursing facilities.
• Implement therapy sessions to improve patient mobility, strength, and overall well-being.
• Monitor patient progress and adjust treatment plans accordingly.
• Collaborate with healthcare teams to ensure comprehensive patient care.
• Document all therapy activities accurately and maintain detailed patient records.
• Ensure compliance with all safety and regulatory standards.

Required Qualifications
• Valid Physical Therapist license for the state of California.
• Resume submission and license verification documentation.
• Ability to provide references (minimum of 3).
• Must pass a criminal background check covering both county and federal levels (7 years or more).
• Nationwide sex offender and HHS/OIG searches cleared.
• Valid social security number trace.
• Current physical exam including TB test (PPD, Quantiferon, or proof of past positive PPD with chest X-ray).
• Negative drug screen results.
• Valid Fire Safety Card for California (issued within the last 4 years).
• Covid testing within 72 hours of start date with results issued at least 48 hours prior.

Preferred Experience / Skills
• Recent experience in a skilled nursing facility setting preferred.
• New graduates are encouraged to apply.
