Key Responsibilities
• Provide clinical social work services within a hospital setting, focusing on patient needs and discharge planning.
• Collaborate with medical staff and multidisciplinary teams to develop and implement patient care plans.
• Document assessments, interventions, and progress in compliance with hospital and regulatory standards.
• Support patients and families through emotional and psychosocial challenges related to health conditions.
• Ensure timely and accurate communication of patient information and care coordination.

Required Qualifications
• Licensed Certified Social Worker (LCSW) in Georgia.
• Minimum of 2 years of experience in a healthcare or hospital environment.
• Proven experience working in an acute hospital setting.
• Ability to work night shifts and flexibility to meet scheduling demands.

Preferred Experience / Skills
• Experience working in behavioral health or mental health settings.
• Strong interpersonal and communication skills.
• Ability to handle complex casework with sensitivity and professionalism.

Candidates must be willing to work the required holidays, including two major holidays (Thanksgiving, Christmas, New Year’s) and one minor holiday (Day after Thanksgiving, Christmas Eve, or New Year’s Eve).
