Key Responsibilities
- Provide comprehensive physical therapy services to patients in various clinical settings.
- Assess patients' physical conditions, develop treatment plans, and implement therapy interventions.
- Monitor patient progress and adjust treatments accordingly.
- Complete detailed documentation of patient care and progress.
- Collaborate with healthcare teams to optimize patient outcomes.
- Maintain compliance with all licensing and safety regulations.
Required Qualifications
- Valid State license as a Physical Therapist.
- Current Physical Therapy license verification.
- Must complete and submit prescreening, resume, and license verification documents.
- Minimum 3 references available upon request.
- Clear background checks including criminal history (7-year span), sex offender registry, and federal searches.
- Social Security Number trace and negative drug screen results required.
- Valid Physical Fire Safety Card (every 4 years).
Preferred Experience / Skills
- Prior experience with Rehab Optima software.
- Experience working in SNF (Skilled Nursing Facility) settings highly preferred.
- Familiarity with credentialing processes and documentation requirements.
