Key Responsibilities

• Assist licensed physical therapists in delivering patient care in a Skilled Nursing Facility (SNF) setting.
• Conduct therapeutic exercises and modalities as directed by the supervising therapist.
• Document patient progress and communicate effectively with the care team.
• Maintain compliance with all licensing and facility requirements.
• Adhere to patient safety and confidentiality guidelines.

Required Qualifications

• Valid Massachusetts State License as a Physical Therapy Assistant.
• Current professional resume and license verification attached.
• Complete prescreen questionnaire submitted with application.
• Criminal background checks for all counties lived and worked in for the past 7 years.
• Nationwide background checks including sex offender registry, HHS/OIG, and GSA searches.
• Social Security Number trace and other credentialing requirements.
• 8 hours of dementia-specific training.

Preferred Experience / Skills

• Previous SNF experience highly preferred.
• Knowledge of Massachusetts criminal background process.
• Ability to work effectively in a team environment.
