Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility setting.
• Assess patient conditions and develop individualized treatment plans.
• Document patient progress and maintain accurate records.
• Collaborate with healthcare team members to ensure optimal patient care.
• Follow safety protocols and adhere to regulatory guidelines.
• Ensure compliance with facility policies and licensing requirements.

Required Qualifications

• Valid state license as a Physical Therapist.
• Graduation from an accredited physical therapy program.
• Ability to pass a Covid-19 test prior to the start date with results issued 48 hours before start.
• Prescreening completed and attached with application, including license verification, resume, and references.

Preferred Experience / Skills

• Prior experience working in a skilled nursing facility (SNF).
• New grads are welcome to apply.
• Experience with credentialing processes and background checks.
