Job Title LVN / LPN - LTC/SNF

Key Responsibilities • Provide high-quality nursing care to residents in a long-term care setting, including administering medications, monitoring vital signs, and documenting patient information. • Develop and implement individualized care plans to meet the unique needs of each resident, in collaboration with interdisciplinary teams. • Educate patients and their families on proper wound care, infection prevention, and other health-related topics. • Participate in daily reports and care conferences to ensure seamless communication and coordination among care team members.
Required Qualifications • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of NH. • Valid CPR certification. • Ability to lift, transfer, and position residents as needed. • Strong communication and interpersonal skills.
Preferred Experience / Skills • Previous experience working in a long-term care or skilled nursing facility. • Knowledge of medical terminology and ability to read and interpret medical records. • Experience with electronic medical records (EMRs) and other healthcare technology.
