• LVN / LPN - Long-Term Acute Care (LTAC)

• Full-time, Night shifts

Key Responsibilities

• Provide direct patient care in a long-term acute care setting, including subacute rehab, memory care, pulmonary, and ventilator units.
• Monitor patient condition, recognize changes, and respond appropriately to emergencies and alarms.
• Administer medications, treatments, and wound care as per physician instructions.
• Document patient assessments, care plans, and treatments accurately and promptly.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Ensure compliance with facility policies and safety standards, including badge-in/out procedures for dementia units.

Required Qualifications

• Valid LVN or LPN license in the state of Maryland.
• Minimum of one year experience in a hospital, LTC, or LTAC setting.
• Current ACLS and BLS certifications.
• Ability to work night shifts and adapt to a fast-paced environment.
• Strong assessment and communication skills.

Preferred Experience / Skills

• Experience caring for patients with respiratory issues, traachestomy, ventilator support, or dementia.
• Prior experience in subacute rehab, memory care, or pulmonary units.
• Knowledge of clinical interventions for complex respiratory and chronic illnesses.
