Key Responsibilities:
• Provide high-quality nursing care to residents in a long-term care setting, including administering medications, monitoring vital signs, and documenting patient information. • Develop and implement individualized care plans to meet the unique needs of each resident, in collaboration with the interdisciplinary care team. • Educate residents and their families on proper care techniques, disease management, and healthy lifestyle habits. • Maintain accurate and up-to-date patient records, in accordance with regulatory requirements. • Participate in quality improvement initiatives to optimize resident outcomes and facility operations.
Required Qualifications:
• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of Alabama. • Minimum of 1 year of experience in a long-term care setting, preferably in LTC/SNF. • Basic Life Support (BLS) certification. • Ability to pass a background check and obtain necessary clearances.
Preferred Experience / Skills:
• Experience with electronic medical records (EMRs) and other clinical software. • Knowledge of gerontological nursing principles and practices. • Bilingual or multilingual skills.
