Key Responsibilities
• Providing direct patient care in long-term care and skilled nursing facilities
• Administering medications and treatments as prescribed by physicians
• Monitoring patient health and documenting observations accurately
• Collaborating with interdisciplinary teams to ensure quality patient care
• Assisting with patient assessments and care planning
• Ensuring compliance with healthcare protocols and safety standards
Required Qualifications
• Valid LVN / LPN license in the state of Massachusetts
• Recent experience in long-term care or skilled nursing facilities
• Strong communication and patient care skills
• Ability to work independently and as part of a team
Preferred Experience / Skills
• Bilingual abilities are a plus
• Knowledge of electronic health records (EHR) systems
• Prior work experience in a SNF or LTC setting
