Key Responsibilities

• Provide social work services within an adult day care program, including individual and group counseling.
• Conduct assessments addressing social, emotional, cognitive, and ethnic factors to develop personalized care plans.
• Assist participants in accessing community and financial resources to support ongoing care.
• Perform social work assessments and interventions in both the center and participant homes, utilizing a moderate amount of in-field work.
• Collaborate with an interdisciplinary team to ensure continuity of care and support for participants.
• Manage a flexible schedule for home visits within the designated radius, with mileage reimbursement for travel between work locations during work hours.

Required Qualifications

• Valid WA LICSW, WA LSWAIC, or WA LASW license (temporary licensure accepted).
• BLS certification.
• Minimum of 2 years of social work experience with a geriatric population.
• Ability to travel within Snohomish County for home assessments.
• Strong interpersonal skills and ability to work independently.

Preferred Experience / Skills

• Experience working in outpatient or adult day care settings.
• Knowledge of community resources and care planning.
• Comfortable with flexible scheduling and autonomous work planning.
