Key Responsibilities
- Provide nursing care to residents in long-term care and skilled nursing facilities.
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor patient health status and report changes to the healthcare team.
- Assist with activities of daily living and ensure residents' comfort and safety.
- Document patient care accurately and thoroughly.
- Collaborate with interdisciplinary teams to ensure quality care delivery.
Required Qualifications
- Valid LVN or LPN license in the state of West Virginia.
- Experience working in LTC or SNF settings preferred.
- Basic life support (BLS) certification.
- Strong communication and patient care skills.
- Ability to follow detailed care plans and protocols.
Preferred Experience / Skills
- Previous experience in long-term care or skilled nursing facilities.
- Knowledge of state and local healthcare regulations.
