Key Responsibilities
- Provide skilled nursing care to residents in long-term care or skilled nursing facilities.
- Assist with patient assessments, medication administration, and treatment plans.
- Collaborate with the healthcare team to ensure quality patient care.
- Maintain accurate and complete documentation in electronic medical records.
- Follow facility policies, procedures, and safety protocols.
- Support residents with activities of daily living as needed.
Required Qualifications
- Valid LVN or LPN license.
- Basic life support (BLS) certification.
- Experience working in long-term care or skilled nursing facilities preferred.
- Proficiency with electronic medical records, preferably PCC.
- IV certification (preferred but not required).
Preferred Experience / Skills
- Prior experience in a SNF or LTC setting.
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills.
