Key Responsibilities

• Provide occupational therapy services to clients in a skilled nursing facility setting.
• Evaluate patients’ functional needs and develop personalized treatment plans.
• Implement therapy interventions aimed at improving client independence and quality of life.
• Complete documentation and progress notes accurately and in a timely manner.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Ensure compliance with all licensing, credentialing, and safety protocols.

Required Qualifications

• Valid state license as an Occupational Therapist.
• Must provide a current resume and license verification upon submission.
• Covid-19 testing completed within 72 hours prior to start date, with results issued 48 hours prior.
• Willingness to work in a skilled nursing facility environment.
• New graduates are welcome to apply.

Preferred Experience / Skills

• Prior SNF (Skilled Nursing Facility) experience preferred.
• Ability to complete and pass all credentialing and background checks.

Credentialing & Pre-employment Requirements

• Credentialing checklist includes:
  • State license verification
  • Resume submission
  • Three professional references
  • Criminal background checks (county, federal, multi-state, Nationwide Sex Offender search for the past 7 years or more)
  • Federal healthcare sanctions check (HHS/OIG) and GSA search
  • Social Security Number trace
  • TB screening (PPD, Quantiferon, or proof of positive PPD + chest X-ray)
  • SAM (System for Award Management) registration
  • Oregon State Background Check via CRIMS
