Key Responsibilities
• Provide physical therapy services to patients,Assess patient needs and develop personalized treatment plans
• Implement and adjust therapy interventions to improve patient outcomes
• Document patient progress and maintain accurate medical records
• Collaborate with healthcare team to coordinate patient care
• Ensure compliance with all relevant safety and regulatory standards

Required Qualifications
• Current state physical therapy license
• Graduation from an accredited physical therapy program
• Strong patient assessment and treatment skills
• Excellent communication and interpersonal skills

Preferred Experience / Skills
• Prior experience in a SNF or long-term care setting preferred
• Knowledge of Medicare regulations and documentation standards
