Key Responsibilities
- Provide direct, individualized patient care in a home setting for inpatients on an acute care level.
- Implement and follow personalized care plans for each patient, utilizing technology and remote clinical coordination.
- Conduct home visits to monitor, assess, and document patient health status, ensuring quality and safety standards are maintained.
- Collaborate with a multidisciplinary healthcare team to deliver therapeutic interventions and coordinate patient care remotely.
- Apply the nursing process within a team nursing model to deliver comprehensive care in the patient's home environment.
- Maintain accurate documentation of patient care activities and progress.
Required Qualifications
- Minimum of 3 years of critical care, acute care, or medical-surgical RN experience.
- BSN degree required.
- Valid VA or Compact State nursing license at the time of submission.
- Ability to work rotating 12-hour shifts (7 am - 7 pm and 11 am - 11 pm).
- Mileage reimbursement at IRS rates.
Preferred Experience / Skills
- Experience with home health or remote patient care services.
- Strong documentation and communication skills.
- Proficiency with healthcare technology and remote clinical tools.
