Key Responsibilities
- Complete and accurately fill out OASIS documentation in compliance with agency policies, state, and federal regulations, ensuring timeliness and accuracy.
- Assess patient and caregiver needs using the nursing process to develop personalized, evidence-based care plans.
- Serve as Case Manager, actively managing caseloads and visit schedules to meet productivity standards.
- Implement medical plans of care, nursing interventions, and procedures to deliver safe, effective, and high-quality patient care.
- Maintain up-to-date technical skills, including IV therapy, wound VAC, and complex wound care, ensuring infection control standards are upheld.
- Collaborate with LPNs to follow the care plan, delegate tasks appropriately, and review visit documentation for accuracy and completeness.
- Evaluate patient responses to care, monitor outcomes, and adjust plans as necessary to ensure quality and patient satisfaction.
- Document nursing care comprehensively and accurately to facilitate ongoing communication within the healthcare team.
- Educate patients and caregivers on medications, treatment options, discharge instructions, and reportable conditions.
- Identify interdisciplinary needs such as physical therapy or social work, documenting and communicating these needs promptly.
- Complete all required forms (e.g., HHABN, Medicare Notice of Non-Coverage) accurately and in a timely manner.
- Conduct patient visits efficiently, ensuring supervisory visits are scheduled separately when required, and communicate with scheduling staff proactively.
- Assess need for home health aide services, develop, and coordinate plans of care, and provide ongoing supervision.
- Report incidents (e.g., infections, injuries) promptly to Quality Improvement personnel.
- Coordinate patient discharges and transfers, ensuring all paperwork is processed compliant with department and regulatory standards.
- Collaborate with physicians, providers, team members, and vendors to support safe and effective patient care.
- Participate in monthly peer reviews, providing constructive feedback and accepting evaluations professionally.
- Organize and facilitate case conferences with management and team members to promote effective communication.
- Advocate for patient care needs to ensure services are tailored to individual family and patient preferences.
Required Qualifications
- Valid RN license in Vermont or eligible for licensure.
- Proven experience with hospice care or home health nursing preferred.
- Knowledge of OASIS documentation requirements and compliance standards.
- Strong assessment, care planning, and patient education skills.
- Ability to work collaboratively within a multidisciplinary team.
- Comfortable with high-tech skills such as IV therapy, wound VAC, and infection control procedures.
Preferred Experience / Skills
- Prior hospice or palliative care experience.
- Familiarity with McKesson documentation processes and electronic medical records.
- Excellent communication and organizational skills.
- Ability to manage a diverse caseload efficiently in a fast-paced environment.
