- Full-time, Contract (13-week assignment), Days shift
Key Responsibilities
- Evaluate and treat residents to improve independence in activities of daily living (ADLs).
- Develop and implement individualized treatment plans tailored to each patient’s needs.
- Collaborate with interdisciplinary team members including physical therapists, speech therapists, nurses, and caregivers.
- Provide caregiver and family training to support safe and successful transitions.
- Document patient progress using electronic medical records for efficient workflow.
- Foster a positive, team-based, and compassionate therapy environment.
Required Qualifications
- Active Occupational Therapist (OT) license in good standing in Pennsylvania.
- Valid CPR certification.
- Active and clear state and federal background checks, including fingerprinting if not a resident of PA for two consecutive years.
- Ability to pass TB or PPD blood test, and provide MMR and Hep B status.
- Proficiency in documentation and communication skills.
Preferred Experience / Skills
- Experience in skilled nursing, rehabilitation, or older adult care.
- Strong problem-solving and interpersonal skills.
- Ability to work effectively within a team-oriented setting.
