- Full-time, contract, 13-week assignment, days shift, onsite
Key Responsibilities
- Evaluate and treat residents to enhance independence in activities of daily living (ADLs).
- Develop and implement individualized treatment plans tailored to each patient's needs.
- Collaborate with a multidisciplinary team including physical therapists, speech therapists, nurses, and caregivers.
- Provide training and education to caregivers and families to support safe and effective care transitions.
- Document patient progress accurately using electronic medical records (Net Health EMR).
- Foster a positive, team-oriented environment grounded in compassion and teamwork.
Required Qualifications
- Active Occupational Therapist (OT) license in good standing in Pennsylvania.
- Experience in skilled nursing, rehab, or older adult care preferred.
- Strong communication, problem-solving, and interpersonal skills.
- Active CPR certification.
- Ability to pass state and federal background checks, fingerprinting (if applicable), and health screenings.
Preferred Experience / Skills
- Previous experience in a retirement or skilled nursing facility.
- Familiarity with EMR documentation tools such as Net Health.
- Experience working with elderly populations.
