Key Responsibilities
• Provide occupational therapy services to residents in a skilled care setting
• Develop and implement personalized treatment plans to enhance residents' functional abilities
• Collaborate with interdisciplinary team to ensure holistic patient care
• Document treatment progress and maintain accurate patient records
• Assist in establishing and strengthening the rehab team

Required Qualifications
• Valid state-specific occupational therapist license
• Current registration and credentials to practice occupational therapy
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Preferred Experience / Skills
• Experience working in a skilled nursing or assisted living environment
• Knowledge of rehab protocols and resident care procedures

Additional Details
• Compensation: $40–$47 per hour
• Schedule: 32 hours per week, Monday through Friday, no weekends
• Facility: Fairhaven Christian Retirement Center, a reputable IL/AL and Intermediate Care community in Rockford
