Key Responsibilities
- Provide hospice nursing care to patients in their homes across Thurston, Mason, and Lewis counties.
- Manage a caseload with up to two patient admissions per day, ensuring compliance with all documentation and care standards.
- Conduct patient visits, with the possibility of visiting up to six patients daily depending on workload and productivity points.
- Document all patient interactions using HomeCare HomeBase and Oasis electronic systems, ensuring timely and accurate reports.
- Coordinate care with interdisciplinary teams to meet patient needs and uphold care quality.
- Maintain professional licensure and certifications, including BCLS, and adhere to compliance and safety protocols.
Required Qualifications
- Minimum of 1 year of recent skilled home health nursing experience within the last 3 years.
- Valid WA RN license or compact license.
- Current BCLS certification.
- Reliable personal transportation to travel within assigned counties, with occasional travel up to a 50+ mile radius.
Preferred Experience / Skills
- Experience working in hospice or geriatric care settings.
- Ability to manage a dynamic caseload with efficiency and compassion.
- Knowledge of electronic medical records, specifically HomeCare HomeBase and Oasis systems.
