Key Responsibilities
- Conduct initial and annual foot assessments for patients with diabetes, including reviewing medical history and medications.
- Examine foot structure, circulation, skin, and nails; document findings accurately.
- Provide routine foot care such as nail trimming, filing, and management of calluses, corns, and ingrown nails.
- Inspect footwear for safety and proper fit; educate patients on foot health and prevention strategies.
- Refer patients to medical providers when further evaluation or treatment is necessary.
- Maintain detailed documentation of services provided in the electronic health record.
Required Qualifications
- Current unencumbered Oregon RN license
- Healthcare Provider BLS certification
- Strong understanding of comprehensive foot and diabetes care
Preferred Experience / Skills
- Certification as a Diabetes Educator (CDE or CDCES)
- Prior teaching or patient education experience in a healthcare setting
- Minimum of three (3) years clinical experience as an RN or RD, with emphasis on diabetes management or chronic disease care
