Key Responsibilities
- Conduct psychosocial assessments and develop treatment plans for adult patients.
- Lead group therapy sessions and facilitate support and family meetings.
- Collaborate with the treatment team to coordinate patient care.
- Document all patient interactions and treatment activities accurately in electronic health records.
- Work primarily with adults diagnosed with conditions such as dementia, depression, anxiety, and suicidal ideation or aggression.
Required Qualifications
- Licensed Social Worker (LSW or LCSW), LPC, or LCPC.
- Minimum of 2 years of experience as a social worker in an acute care setting.
- Valid CPR certification.
- Proficiency with Epic electronic charting system.
Preferred Experience / Skills
- Experience working with patients with mental health diagnoses, including dementia and mood disorders.
- Strong communication and teamwork skills.
