Job Title: Physical Therapist

Location: Danbury, CT

Schedule / Job Type: Full-time, Day shifts

Key Responsibilities:

• Provide skilled physical therapy services to patients in a skilled nursing facility (SNF).
• Assess patient needs, develop treatment plans, and modify interventions as needed.
• Document patient progress and maintain accurate medical records.
• Collaborate with healthcare team members to optimize patient outcomes.
• Ensure compliance with all regulatory and safety standards.

Required Qualifications:

• Valid license to practice as a Physical Therapist in the state of Connecticut.
• Minimum of 1 year of professional SNF experience.
• Strong clinical assessment and documentation skills.
• Ability to work independently and as part of a team.

Preferred Experience / Skills:

• Experience working specifically in skilled nursing facilities (SNF).
• Familiarity with Medicare and Medicaid documentation requirements.
• Excellent communication and interpersonal skills.
