Job Title: Therapist - Physical Therapist
Location: Liberty, IN
Schedule / Job Type: Days, Full-time

Key Responsibilities:

• Assess and develop patient treatment plans to improve mobility and function.
• Provide therapy sessions adhering to patient's individual needs and clinical guidelines.
• Document patient progress and communicate with healthcare teams.
• Ensure all treatments comply with regulatory and safety standards.
• Maintain accurate and complete clinical records and licenses.

Required Qualifications:

• Valid state licensure as a Physical Therapist.
• Must pass credentialing checks including license verification and background screenings.
• Ability to perform detailed assessments and develop appropriate treatment plans.
• Strong communication skills for effective patient and team interactions.

Preferred Experience / Skills:

• Previous experience working in Skilled Nursing Facility (SNF) setting preferred.
• Recent graduates are encouraged to apply.
• Covid-19 test within 72 hours of start date, with results issued at least 48 hours prior.

• Nationwide sex offender search (7+ years)

• Social Security Number trace (1 step)
• Proof of TB screening (PPD or Quantiferon, or positive PPD and chest X-ray)
• SAMS registration
• State background check
