Key Responsibilities

• Provide skilled physical therapy services to patients in a skilled nursing facility.
• Develop and implement individualized treatment plans based on patient assessments.
• Monitor patient progress and adjust therapy plans accordingly.
• Collaborate with healthcare team members to ensure optimal patient care.
• Maintain accurate documentation of therapy sessions and patient records.
• Comply with all regulatory and facility guidelines.

Required Qualifications

• Valid state licensure as a Physical Therapist.
• Ability to complete and submit pre-screening documentation.
• CPR certification.
• Physical ability to perform therapy duties.
• COVID-19 test within 72 hours prior to start with results issued 48 hours before shift.

Preferred Experience / Skills

• SNF (Skilled Nursing Facility) experience.
• Experience with Rehab Optima is a plus.
• Knowledge of dementia care is advantageous.
