Key Responsibilities
- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting.
- Work across multiple units, including memory care and sub-acute.
- Develop and implement individualized treatment plans.
- Collaborate with healthcare teams to ensure optimal patient outcomes.
- Monitor and document patient progress and adjust therapy as needed.
Required Qualifications
- Valid State License as a Speech-Language Pathologist
- Certification in Speech-Language Pathology (if applicable)
- Experience working in SNF or similar healthcare settings
- Strong interpersonal and communication skills
Preferred Experience / Skills
- Prior experience with memory care and sub-acute units
- Knowledge of Medicaid and Medicare guidelines
- Ability to work effectively in a team environment
