Schedule / Job Type:
Full-time, Days shifts, Onsite

Key Responsibilities:

• Provide home health nursing care to patients, managing a caseload of 5-6 patients daily.
• Assess, plan, and implement patient care plans, including wound care, medication management, and disease control.
• Document patient interactions and clinical notes accurately using the EPIC EMR system remotely.
• Collaborate with the healthcare team to ensure quality patient care and communicate patient progress.
• Manage patient needs independently while maintaining compliance with all department and facility policies.

Required Qualifications:

• Active IL RN or LPN license; active or pending application.
• Current BLS certification.
• Valid driver’s license and insurance.
• Last four digits of SSN and middle initial required for submission.
• Understanding of Medicare/Medicaid documentation standards.
• Ability to work independently and provide excellent patient care.

Preferred Experience / Skills:

• Home health and wound vac experience preferred.
• Experience with EPIC REMOTE client EMR system.
• Knowledge of wound care, medications, and disease management.
