Key Responsibilities

• Assist licensed therapists in providing patient care in a skilled nursing facility.
• Implement treatment plans under supervision, including therapeutic exercises and activities.
• Monitor patient progress and report outcomes to supervising therapists.
• Maintain accurate documentation of patient treatments and progress notes.
• Adhere to all safety and confidentiality regulations.

Required Qualifications

• Valid state license as a Physical Therapy Assistant.
• Relevant educational credentials in Physical Therapy Assisting.
• Ability to work effectively in a team environment.
• Strong communication and interpersonal skills.
• Compliance with all credentialing requirements, including license verification and background checks.

Preferred Experience / Skills

• Prior experience in a skilled nursing facility (SNF).
• Familiarity with federal, state, and facility-specific regulatory requirements.
• Experience with dementia care is a plus.

Additional Credentialing Requirements
Candidates must complete and submit the following prior to onboarding:

• Prescreen form (attached)
• Resume and license verification documents
• References (3)
• Criminal background checks (state, federal, multi-state, nationwide sex offender) for the past 7 years
• Background check through state-specific and national databases
• Social Security Number trace
• TB testing (PPD or Quantiferon) with proof of positive PPD and chest X-ray if applicable
• Additional training in dementia care (minimum 8 hours)
