Key Responsibilities
- Provide skilled physical therapy services to patients in a skilled nursing facility (SNF).
- Assess patients' physical needs and develop individualized treatment plans.
- Implement therapy interventions to promote optimal patient outcomes.
- Monitor patient progress and adjust treatment plans accordingly.
- Document patient assessments, treatments, and progress notes in compliance with facility protocols.
- Collaborate with healthcare team members to coordinate patient care.
Required Qualifications
- Valid physical therapy license in the state of Connecticut.
- Professional experience working in a skilled nursing facility (SNF).
- Strong clinical assessment and treatment planning skills.
- Excellent communication and documentation abilities.
Preferred Experience / Skills
- Prior experience working in a long-term care or SNF setting.
- Knowledge of relevant healthcare regulations and compliance standards.
