Key Responsibilities
• Provide comprehensive physical therapy assessments and develop individualized treatment plans for patients.
• Implement therapeutic interventions to improve mobility, strength, and function.
• Monitor patient progress and adjust treatment plans as needed.
• Document clinical notes accurately and maintain patient records in compliance with regulations.
• Collaborate with healthcare teams to ensure optimal patient care.
• Adhere to all licensing and facility policies throughout treatment delivery.

Required Qualifications
• Valid state license as a Physical Therapist.
• Degree from an accredited physical therapy program.
• Ability to pass credentialing checks, including criminal background, license verification, and references.
• Prescreen and documentation including resume, license, and credentials are mandatory for submission.

Preferred Experience / Skills
• Prior experience working in Skilled Nursing Facilities (SNF) preferred.
• Knowledge of geriatric patient care highly desired.
• Strong documentation and communication skills.
