- Full-time, Days (Monday to Friday, 8:30 am - 5:00 pm)
Key Responsibilities
- Evaluate the appropriateness of ongoing hospice care for assigned patients in collaboration with hospice leadership and the medical director.
- Conduct face-to-face assessments in patients' homes to determine eligibility for hospice services.
- Coordinate and communicate with hospice physicians to ensure proper documentation and recertification of patient care.
- Provide consultative support to case managers regarding chronic conditions and symptom management.
- Assist with smooth transitions for patients entering hospice care, including assessment, documentation, and coordination of services.
- Participate in interdisciplinary team meetings, develop and update care plans, and contribute to quality assurance activities.
Required Qualifications
- Current Nurse Practitioner license or Physician Assistant certification in California.
- Minimum of 1 year of recent professional nursing experience or equivalent clinical practice.
- Knowledge of hospice care protocols, documentation, and interdisciplinary collaboration.
- Experience with electronic health records and telecommunication devices.
- Valid California driver's license and auto insurance meeting agency requirements.
- Current CPR certification.
Preferred Experience / Skills
- Demonstrated program development and teaching abilities.
- Previous experience in hospice or palliative care settings.
- Ability to work independently and as part of a multidisciplinary team.
- Flexible, self-directed, and highly motivated.
