Therapist - Physical Therapist
Location: Aberdeen, WA
Schedule / Job Type: Full-time, Days

Key Responsibilities:

• Provide skilled physical therapy services to patients in a skilled nursing facility setting
• Develop and implement patient-specific treatment plans to improve mobility and overall well-being
• Document patient progress and communicate with interdisciplinary team members
• Ensure compliance with facility policies and state regulations
• Maintain accurate and timely medical records

Required Qualifications:

• Valid state license as a Physical Therapist
• Certified and in good standing in the state of Washington
• Experience working in a skilled nursing facility preferred
• Knowledge of rehab protocols and therapy best practices
• Ability to complete credentialing requirements effectively

Preferred Experience / Skills:

• Prior experience with Rehab Optima or similar therapy software
• Strong organizational and communication skills
• Ability to work independently and as part of a team

Credentialing Checklist (to be completed prior to submission):

• State license verification
• Resume and references (3)
• Criminal background checks (county, federal, multi-state, sex offender) for the past 7 years
• HHS/OIG, GSA, and Social Security Number Trace
• Two-step PPD or Quantiferon test/Proof of positive PPD with Chest X-ray
• Washington State background check with fingerprints (facility run)
