Registered Nurse - Hospice
Location: Gunnison, CO
Schedule / Job Type: Full-time, Days, Onsite

Key Responsibilities:

• Provide patient care and case management using the nursing process and individualized Plan of Care for hospice and palliative care patients.
• Administer skilled nursing services to patients requiring intermittent professional nursing care in accordance with departmental policies and procedures.
• Conduct initial evaluation visits, regularly re-evaluate patient needs, and initiate or revise the care plan accordingly.
• Perform specialized nursing procedures, including preventive and rehabilitative care.
• Document clinical notes and progress, coordinate services, and communicate patient condition updates to physicians and team members.
• Counsel patients and families to meet nursing and related needs, and participate in in-service training programs.
• Supervise and educate other nursing personnel.

Required Qualifications:

• Valid Registered Nurse license in the state of Colorado.
• Experience in hospice, palliative care, or home health nursing preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
• Knowledge of nursing standards and practices related to end-of-life care.

Preferred Experience / Skills:

• Prior experience in hospice or palliative care settings.
• Certification in hospice or palliative nursing (e.g., CHPN).
• Excellent patient and family counseling skills.
