Key Responsibilities

  • Effectively completes OASIS documentation in accordance with agency policies, regulations, and updates.
  • Assesses patient and caregiver needs using the nursing process to ensure high-quality, evidence-based care.
  • Develops and communicates realistic, patient-centered care plans, including patient and caregiver participation.
  • Manages assigned patient caseloads, coordinating visits to meet productivity standards.
  • Implements medical plans of care, nursing interventions, and procedures to promote safe and effective patient outcomes.
  • Maintains proficiency in high-tech skills such as IV therapy, wound VAC, and complex wound care, ensuring infection control standards are upheld.
  • Collaborates with LPNs to ensure adherence to care plans, delegates tasks appropriately, and reviews visit documentation.
  • Evaluates the effectiveness of care plans, patient responses, and makes adjustments to meet patient needs.
  • Ensures accurate and up-to-date documentation to support ongoing quality of care and facilitate team collaboration.
  • Adheres to agency protocols for visit documentation, including visit locking, follow-up notes, and completion.
  • Provides patient and caregiver education on medication, treatment options, discharge instructions, and reportable conditions.
  • Identifies interdisciplinary needs and communicates with relevant providers (physical therapy, social work, etc.) efficiently.
  • Completes agency, state, and federal forms accurately and timely, including Medicare notices and other essential documentation.
  • Conducts patient visits efficiently, coordinating with scheduling for appropriate timing and follow-up.
  • Assesses patient needs for Home Care Agency services, develops, and supervises care plans in compliance with regulations.
  • Reports patient incidents (infections, injuries, etc.) promptly to quality improvement personnel.
  • Coordinates safe discharge and transfer processes, ensuring all paperwork and information are properly handled.
  • Collaborates with physicians, healthcare team members, vendors, and family to ensure seamless and safe patient care.
  • Participates in monthly peer review audits, providing and receiving feedback constructively.
  • Organizes case conferences with team members and managers to enhance communication and care coordination.
  • Advocates proactively for patient care to ensure personalized, high-quality services tailored to each patient and family.

Required Qualifications

  • Valid RN license in Vermont or appropriate state licensure.
  • Proven experience with hospice or home health care preferred.
  • Knowledge of OASIS documentation and federal/state regulations governing home health.
  • Proficiency in high-tech nursing skills such as IV therapy, wound VAC, and complex wound care.
  • Strong collaborative skills to work effectively with interdisciplinary teams.
  • Excellent documentation and communication skills.

Preferred Experience / Skills

  • Previous hospice or palliative care experience.
  • Familiarity with McKesson or other electronic health records systems.
  • Experience managing complex wound care and high-tech interventions.
  • Certification in hospice and palliative care (preferred but not required).
