Key Responsibilities

• Provide comprehensive physical therapy services to patients based on physician referrals.
• Complete required documentation in coordination with the facility prior to start.
• Ensure compliance with licensing and credentialing requirements.
• Follow established treatment plans to improve patient outcomes.
• Collaborate with healthcare team members to support patient care.
• Maintain all documentation tracking and submit weekly billing as per cycle.

Required Qualifications

• Active license as a Physical Therapist.
• Experience in setting up and managing physical therapy treatments.
• Ability to complete documentation accurately and timely.
• Credentialed for contract assignment and credentialing requirements completed.

Preferred Experience / Skills

• Prior experience working in acute care or outpatient settings.
• Familiarity with facility equipment and treatment protocols.
• Strong communication and team collaboration skills.
