Key Responsibilities

• Provide complex end-of-life care, symptom management, and patient education in hospice settings.
• Conduct comprehensive assessments, develop, and participate in goals of care discussions with patients and families.
• Administer medications via IV, subcutaneous, oral, sublingual, rectal, and other routes, ensuring pain and symptom control.
• Manage and maintain patients' advanced equipment such as CADD pumps, BiPap, CPAP, and IV infusion devices.
• Document care and coordinate with interdisciplinary teams using Net Smart EMR.
• Cover occasionally for on-call teams, providing call coverage as needed.

Required Qualifications

• At least 3 years of clinical experience in community hospice or end-of-life care.
• Current BLS certification.
• Strong critical thinking skills, with experience in symptom management and education.
• Ability to manage complex symptom protocols and use advanced equipment.
• Reliable transportation; personal or rental vehicle preferred, equipped for winter conditions.

Preferred Experience / Skills

• Experience with in-home hospice, SNF, or ALF settings.
• Comfortable conducting goals of care discussions and education.
• Proficiency with EMR systems, specifically Net Smart.
