Key Responsibilities
- Conduct comprehensive assessments of patients and caregivers to develop and communicate individualized care plans in compliance with state, federal, and organizational policies.
- Participate actively in interdisciplinary team meetings to determine and review patient care within designated timeframes.
- Serve as Case Manager for assigned patients, managing caseloads and visit schedules to ensure quality care delivery.
- Implement medical and nursing interventions, including complex wound care, IV maintenance, and symptom management, following safety and infection control standards.
- Delegate appropriate duties to LPNs and oversee their compliance with the care plan.
- Maintain accurate and up-to-date documentation of all nursing care, assessments, and patient interactions to support quality and continuity of care.
- Educate patients and caregivers on medication management, treatment options, discharge instructions, and reportable conditions to facilitate informed decisions.
- Complete required documentation and agency forms such as Medicare notices and other regulatory reports promptly and accurately.
- Coordinate patient discharges, transfers, and ensure appropriate paperwork and communication are completed per policies.
- Collaborate with healthcare providers, vendors, and team members to ensure safe and effective patient care.
- Participate in quality improvement activities, including monthly peer reviews and audits, and provide constructive feedback.
- Engage with insurance case managers by obtaining prior authorizations and providing updates regarding services and care plans.
- Develop and deliver educational presentations to community and facility teams.
- Facilitate patient transfers and ensure all related documentation is processed efficiently.
- Attend bi-weekly team meetings to maintain clear communication and document discussions.
- Advocate proactively for patient care needs to uphold the highest quality standards and personalized care.
Required Qualifications
- Valid RN license in the state of Vermont.
- Current knowledge of hospice standards, pain management, symptom control, and complex wound care.
- Ability to work independently and as part of a multidisciplinary team.
- Strong documentation and communication skills.
- Proficiency in using agency systems and adherence to infection control protocols.
Preferred Experience / Skills
- Experience in hospice or palliative care.
- Knowledge of Medicare and Medicaid regulations related to hospice services.
- BLS/CPR certification.
- Experience with high-tech medical equipment such as wound VACs and IV therapy.
