Job Title

Schedule / Job Type
• Full-time, Day shifts, 5 days a week
Key Responsibilities
• Provide social work services to participants, including one-on-one and group counseling in an outpatient setting.
• Conduct comprehensive social, emotional, cognitive, and cultural assessments to develop individualized care plans.
• Access and coordinate community and financial resources to support participant needs and ensure continuity of care.
• Perform assessments and interventions in various settings, including participant homes within South King County, as needed.
• Collaborate with interdisciplinary team members to support participant wellbeing and treatment goals.
• Maintain documentation in electronic health records using EPIC and adhere to all regulatory and organizational standards.
Required Qualifications
• Valid BLS certification.
• Master’s in Social Work (MSW).
• One of the following social work credentials: WA LICSW, WA LSWAIC, or WA LASW.
• At least 2 years of experience working with the geriatric population.
• Ability to conduct assessments and provide services both in-clinic and in the community.
Preferred Experience / Skills
• Experience working with older adults in a healthcare or community setting.
• Strong documentation and communication skills.
• Ability to work independently with minimal supervision.
