Key Responsibilities
- Conduct comprehensive patient assessments using the nursing process, including OASIS documentation, to determine patient and caregiver needs.
- Develop and communicate individualized care plans aligned with evidence-based practices and regulatory guidelines, ensuring patient and caregiver involvement.
- Serve as Case Manager by managing caseloads and visit schedules to meet agency productivity standards.
- Implement nursing interventions, procedures, and medical plans of care to ensure safe, effective, and high-quality patient care.
- Maintain up-to-date knowledge of high-tech skills such as IV therapy, wound care, and wound VAC management, ensuring infection control standards are upheld.
- Collaborate with Licensed Practical Nurses (LPNs) to ensure care plans are followed, delegate tasks appropriately, and review visit documentation.
- Evaluate patient responses and the effectiveness of care plans, making adjustments as needed to optimize outcomes.
- Accurately document all nursing care and patient interactions in compliance with agency, state, and federal regulations, including timely completion of forms like HHABN and Medicare notices.
- Educate patients and caregivers on medication management, treatment options, discharge instructions, and reportable conditions.
- Identify additional interdisciplinary needs (such as physical therapy or social work), document findings, and communicate them promptly to the relevant team members.
- Coordinate safe patient discharges or transfers, ensuring all paperwork and information are properly processed.
- Communicate and coordinate with healthcare providers, team members, and vendors to support comprehensive, patient-centered care.
- Participate in peer review audits, providing and receiving constructive feedback professionally.
- Lead and document case conferences with the care team to review patient progress and update care plans.
- Advocate for patient needs to ensure personalized, compassionate care tailored to each family’s circumstances.
- Perform assessments for Home Care Agency services, develop care plans with patients/caregivers, and provide ongoing supervision in line with regulations.
- Report patient incidents or adverse events promptly to Quality Improvement personnel.
- Manage timely and compliant documentation of patient visits and related processes, ensuring proper use of agency systems.
- Support interdisciplinary collaboration and participate in team meetings to optimize patient outcomes.
Required Qualifications
- Current and valid Registered Nurse (RN) license in Vermont.
- Proven experience with OASIS documentation and home health care.
- Knowledge of high-tech skills including IV therapy, wound care, and wound VAC management.
- Ability to work nights and handle a fast-paced, caseload-driven environment.
- Strong communication skills, both verbal and written, with attention to detail.
Preferred Experience / Skills
- Prior experience in home health or similar settings.
- Familiarity with agency-specific systems and protocols, including McKesson processes.
- Ability to collaborate effectively with interdisciplinary teams and coordinate complex care plans.
- Bilingual skills or cultural competence is a plus.
