Key Responsibilities

• Provide bedside hospice care for terminally ill patients in home and inpatient settings.
• Conduct comprehensive patient assessments, symptom management, and care planning.
• Administer medications via various routes including IV, subcutaneous, oral, and rectal.
• Participate in goals of care discussions with patients, families, and the care team.
• Maintain accurate clinical documentation using the agency’s EMR system.
• Support families and caregivers with education on end-of-life care and symptom management.

Required Qualifications

• Active Registered Nurse (RN) license in the state of Vermont.
• A minimum of 3 years of experience in community hospice or end-of-life care.
• BLS certification required.
• Excellent critical thinking, assessment, and communication skills.
• Ability to manage complex symptom control and medication administration.

Preferred Experience / Skills

• Prior experience with end-of-life patient management and complex symptom control.
• Comfort participating in advance care planning discussions.
• Reliable transportation and ability to respond to patient needs outside of scheduled visits.
