Schedule / Job Type

• Full-time, Contract (16-32 weeks), Day shifts

Key Responsibilities

• Provide hospice and end-of-life care, including complex symptom management and patient education.
• Participate in goals of care discussions with patients, families, and interdisciplinary teams.
• Administer medications via multiple routes, including IV, subcutaneous, oral, and rectal.
• Conduct comprehensive assessments to manage symptoms and ensure quality patient care.
• Cover call responsibilities occasionally, including on holidays and weekends, as needed.
• Document care details accurately using electronic medical records.

Required Qualifications

• Active registered nurse license in the state of Vermont.
• AHA BLS certification.
• Minimum of 3 years of hospice or community-based nursing experience.
• Proficiency in electronic medical record documentation.
• Experience with end-of-life care, symptom assessment, and medication management.

Preferred Experience / Skills

• Experience with complex symptom management and education in hospice settings.
• Ability to participate in goals of care planning and facilitate family discussions.
• Strong critical thinking skills and effective communication abilities.
