Key Responsibilities
- Monitor and adjust patient visit schedules to optimize productivity and meet patient needs, including rescheduling and communication with supervisors.
- Perform comprehensive assessments (including OASIS and PT) of assigned patients, evaluating psychosocial, educational, safety, and risk factors in accordance with regulations and guidelines.
- Develop personalized care plans aimed at promoting functional independence and safety, with clear patient-specific goals and interventions.
- Provide skilled physical therapy treatments, including therapeutic techniques and patient/caregiver education, ensuring quality care and documenting progress.
- Coordinate patient care with multidisciplinary team members, facilitating effective communication and ensuring optimal outcomes.
- Supervise and direct clinical activities of PTAs, PT students, and home care aides in compliance with regulations and standards of care.
- Complete all required documentation, ensuring accuracy for clinical records, billing, and insurance processing in accordance with policies and regulations.
- Participate in training programs for healthcare professionals, volunteers, and community groups to promote understanding of rehabilitation services.
- Maintain current knowledge of physical therapy practices and standards, contributing to professional growth and service quality.
- Support the marketing and outreach efforts of rehabilitation services through community engagement and health fairs.
- Conduct patient health assessments (e.g., blood pressure, PT/INR) and perform procedures such as wound care and staple/suture removal as needed.
- Communicate scheduling changes promptly with the scheduling team and supervisors.
- Complete regulatory and agency-specific documentation, including OASIS time points, incident reports, and forms such as HHABN and Medicare notices.
- Efficiently manage patient visits, ensuring timely completion and proper documentation, including safe discharges and transfers, following related protocols.
- Assess needs for additional services (such as HCA), develop and coordinate plans of care, and provide ongoing supervision.
- Report patient incidents (infections, injuries) to Quality Improvement staff promptly.
- Participate in peer reviews and provide constructive feedback to support quality improvement initiatives.
Required Qualifications
- Valid state license as a physical therapist
- Current CPR certification
- Experience performing comprehensive patient assessments and developing care plans
- Knowledge of federal and state healthcare regulations and documentation requirements
- Strong communication and supervisory skills
Preferred Experience / Skills
- Experience in home health or outpatient therapy settings
- Familiarity with OASIS documentation and billing processes
- Ability to work independently and as part of a multidisciplinary team
