Key Responsibilities

• Provide skilled physical therapy services to patients in a long-term care or skilled nursing facility setting.
• Assess patient needs, develop treatment plans, and implement therapeutic interventions.
• Document patient progress and update medical records accordingly.
• Collaborate with the interdisciplinary team to ensure optimal patient outcomes.
• Comply with all safety, health, and compliance standards.

Required Qualifications

• Valid Pennsylvania state license or relevant state licensure as a Physical Therapist.
• Current CPR certification.
• Experience in a skilled nursing facility preferred.
• Ability to pass pre-employment screening, including license verification, resume review, and background checks.
• Vaccination status must be provided; candidates requiring exemptions should specify if applicable.
• Covid-19 test within 72 hours prior to start date with results available 48 hours before the start.

Preferred Experience / Skills

• Prior experience working in a skilled nursing or rehabilitation setting.
• Strong documentation and communication skills.
• Knowledge of state and federal healthcare regulations.
