- Full-time, day shifts, 12-hour shifts, 3 days per week
Key Responsibilities
- Provide direct patient care to inpatients within their home environment, following individualized care plans.
- Deliver high-quality nursing interventions using leveraging technology and remote clinical coordination.
- Document patient care accurately and thoroughly according to hospital standards.
- Collaborate with a multidisciplinary healthcare team to ensure safe and effective therapeutic care.
- Implement the nursing process to assess, plan, implement, and evaluate patient care in the home setting.
- Maintain compliance with safety and quality standards while delivering care in patients’ homes.
Required Qualifications
- Minimum of 3 years of critical care, acute care, or medical-surgical RN experience.
- Bachelor of Science in Nursing (BSN) required.
- Valid VA or compact nursing license at the time of submission.
Preferred Experience / Skills
- Experience providing home health or hospital-at-home services.
- Familiarity with remote patient monitoring and technology-enabled care.
