Schedule / Job Type
- Full-time, 12-hour shifts, Days, 3 shifts per week
Key Responsibilities
- Provide nursing care to residents in long-term care or skilled nursing facility setting
- Administer medications and monitor patient conditions
- Document patient progress and assist with care plans
- Collaborate with interdisciplinary team to ensure quality resident care
- Follow all facility policies and procedures to maintain safety and compliance
Required Qualifications
- Valid and unrestricted Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in Colorado
- LTC (Long-Term Care) experience required
- MDS (Minimum Data Set) experience preferred
Preferred Experience / Skills
- Prior work in a skilled nursing or long-term care facility
- Strong documentation skills
- Ability to work effectively in a team environment
