Job Title

Schedule / Job Type
• No Shift / 12 hours per shift / 3 shifts per week (Traveler assignment)

Key Responsibilities
• Collaborate with RN Case Managers to assess patients' psychosocial needs in inpatient or emergency department settings.
• Provide psychosocial support through direct counseling and collaborative visits, demonstrating cultural humility and sensitivity.
• Perform and document psychosocial assessments to inform care planning.
• Serve as a resource for social work team members, assisting with patient needs within the hospital.
• Coordinate educational and outreach services relevant to patient support and recovery.
• Support patients and their families by addressing physical, social, emotional, economic, and environmental concerns.

Required Qualifications
• Master's degree in Social Work (MSW).
• Licensed Clinical Social Worker (LCSW) license in Colorado.
• Minimum of 3 years of clinical experience in a hospital setting.
• Ability to work in a fast-paced environment with minimal supervision.
• Critical thinking skills and decisive judgment.
• Must be vaccinated against influenza (no exemptions).

Preferred Experience / Skills
• Experience working in inpatient or emergency hospital departments.
• Familiarity with outpatient support services and community resource coordination.
