Key Responsibilities

• Provide skilled nursing care to residents in a long-term care or skilled nursing facility.
• Complete and document patient charts, including COVID-related documentation, using PCC EMR.
• Collaborate with a team, including medication aides and registered nurses, to ensure quality patient care during the day shift.
• Follow facility protocols for resident care, including managing patients with dementia and behavioral health conditions.
• Participate in orientation, training, and shadowing shifts to familiarize with facility routines.
• Support resident safety and well-being, adhering to scheduled routines and documentation standards.

Required Qualifications

• Valid LVN or LPN license.
• Experience with electronic medical records (PCC preferred).
• Knowledge of long-term care or skilled nursing protocols.
• Ability to work 12-hour day shifts.
• Strong documentation and patient care skills.

Preferred Experience / Skills

• Experience caring for elderly patients, including those with dementia and behavioral health issues.
• Prior experience in LTC or SNF settings.
• Ability to work well within a team environment.
• Knowledge of managing residents with schizophrenia, bipolar disorder, or paranoia (medication management understanding).
